Who can use our services?
To receive maternity care at Armadale Hospital you need to:
- Live in these postcodes
- Be referred by your general practitioner or Privately Practising Endorsed Midwife.
- Refer yourself using this link.
We encourage you to:
- Be part of the conversation and involved in shared decision making
- Ask questions if someone is not clear
- Speak up if you have concerns
Referrals
We accept referrals from GPs and Privately Practising Endorsed Midwives by email to akg.referrals@health.wa.gov.au.
Send the completed referral form and a copy of the patient's pregnancy booking blood results, dates, first trimester screening and anatomy scan.
High-risk or urgent referrals
For patients requiring an appointment within seven days, contact the Antenatal Clinic Coordinator directly on 9391 2901.
