Armadale Hospital is now welcoming additional maternity patients and providing care as close to home as possible, as part of WA Health’s response to the COVID pandemic.
 
Patients requiring public maternity services from local postcodes 6105, 6106 and 6107 will be offered referrals to Armadale Hospital where patients can expect accessible, integrated and culturally appropriate care including support for Birthing on Country through the Boodjari Yorgas Midwifery Group Practice and Culturally and Linguistically Diverse (CaLD) patients.


 
GPs seeking clinical advice for COVID-positive patients

East Metropolitan Health Service's (EMHS) GP Factsheet: Seeking clinical advice from EMHS for COVID-positive patients (PDF 170KB) provides general information on how general practitioners can access clinical advice for COVID-positive patients who live within the EMHS catchment, which comprises Royal Perth, Bentley, Armadale and Kalamunda hospitals and St John of God Midland Public Hospital. Please only contact these hospitals for patients who reside in the EMHS metropolitan catchment area (PDF 330KB).

 
Central Referral Service

All non-urgent GP referrals to doctor-led outpatient clinics at Armadale Health Service should be sent to the WA Health Central Referral Service (external site).

The following referrals should NOT be sent to the Central Referral Service:

  • referrals for patients who require immediate review (within next seven days)
  • referrals to a private specialist
  • referrals to obstetrics services (see below)
  • referrals to mental health services (see below)
  • referrals to rural outpatient services
  • referrals to allied health outpatient services
  • referrals to non-doctor led outpatient services.

Armadale Kalamunda Referral Office

Armadale Kalamunda Referral Office can be contacted if any queries on referral processes, referral status or direct referrals (verbally accepted by specialist).

For escalation of a referral please forward an updated referral to the Central Referral Service (as above) with new information and reason for escalation. Sending a duplicate referral is not sufficient.

Immediate Referrals (for urgent care within 7 days) should be directed to the Specialist on-Call at Armadale Health Service 9391 2000 and must be accepted verbally prior to direct referral. These can then be emailed to AKG Referral office if approved by the Specialist for URGENT care. Please provide Approvers Name. If not approved as urgent please forward referral to CRS as normal or send patient to ED if required.

Mental health referrals

To refer a patient to our Adult Mental Health Programs, please use the Adult mental health referral (Word 270KB).

Exclusions

Please note that our mental health outpatient centres do not offer programs for:

  • primary marriage counselling
  • intellectual Impairment services
  • drug, alcohol and substance abuse.

To refer a patient to our Older Adult Mental Health Programs, please use the Older Adult Mental Health GP referral form (Word 270KB).

 
Emergency Department referrals

To refer a patient to the Armadale Health Service Emergency Department (ED), please contact the Duty Officer on 9391 2176.

 
Allied health outpatient referrals

To refer a patient to any of our outpatient allied health services, please use the AHS Rehabilitation and Allied Health referral form (PDF 180KB) or in rich text format (RTF 100KB) and email to akg.referrals@health.wa.gov.au.
