The Armadale Kalamunda Group is committed to enabling shared care planning with our local health and social care providers to assist in the management of complex patients with a chronic condition.

The Armadale Kalamunda Group referral database (PDF 1.1MB) has been created to assist local health professionals in improving their management of these patients by increased access to the services available.

If you have any feedback, or think a service should be included, please email AHS.CareCoordination@health.wa.gov.au.

HealthPathways WA

HealthPathways WA (external site) is a free to access website for all health professionals in WA run by the WA Primary Health Alliance (WAPHA) It covers all areas of Western Australia. The site contains condition specific pathways to assist in assessing, managing and referring patients. General Practitioners write the pathways, in consultation with specialists and other health professionals from across the state.

To access the site:

  1. Request log on details by emailing healthpathways@wapha.org.au
  2. Log on to the website https://wa.healthpathways.org.au (external site)

The aim of HealthPathways WA is to ensure patients access the right care at the right time in the right location. Pathways include local health service information, providing health professionals with local referral options. HealthPathways enable a more seamless, effective and complete patient journey by combining clinical and health service information in one place.

National Health Services Directory

Developed and delivered by Healthdirect Australia, the National Health Services Directory (NHSD) (external site) is a source of health and related services information in Australia.

My Community Directory

My Community Directory (external site) provides a common platform for Health and Community Services organisations to list their services online, for free, as well as provide a source of up to date data for planning, referral and research.

My Aged Care

My Aged Care (external site) aims to make it easier for older people, their families, and carers to access information on ageing and aged care, have their needs assessed and be supported to find and access services.

Connect Groups

Contact Groups (external site) allows you to search an online directory of Self Help and Support Groups within Western Australia.
