  • aged care rehabilitation and assessment
  • neurological condition rehabilitation
  • ICU/acute care.

Outpatients

The Physiotherapy outpatient service is co-located with the Community Rehabilitation Service in the Carl Streich Rehabilitation and Sub-acute care building.

Referrals to outpatient physiotherapy services may be made by a medical or nursing practitioner or an allied health professional.

Please email the Referral forms (PDF 80KB) to akg.referrals@health.wa.gov.au or Fax to 9391 2431. For more information contact the Physiotherapy department on 9391 2281.

All referrals for outpatient Physiotherapy will be triaged on receipt and directed to either the Physiotherapy Outpatient Department if physiotherapy only is required or to the Community Rehabilitation Service if multiple profession input is required.

Waiting time for an initial appointment with the Physiotherapy outpatient department will vary according to the current demand for service.

Holders of Department of Veterans Affairs (DVA) cards, some workers compensation cases and privately insured individuals may be able to access private services without delay.

Student placements

The physiotherapy department provides training placement opportunities for undergraduate and postgraduate physiotherapy students.

Patients have the right to decline an appointment with a student physiotherapist and continue on the waitlist until an appointment with a staff member becomes available.
