Community Rehabilitation
Community rehabilitation provides rehabilitative, aged care and specialist services to anyone aged 18 years and above living in Armadale and surrounding suburbs.
It aims to assist you to improve your functional capacity and maximise your independence.
This service is focussed on helping you to meet your rehabilitation goals within a timeframe. We will work with you to regain or retain certain physical and cognitive abilities that will enable you to remain a part of the local community.
Once you have completed your rehabilitative program with us, we will also link you in with other community organisations if you require further support.
Referrals to community rehabilitation
Community rehabilitation accepts referrals for patients who require rehabilitative therapy to prevent hospital presentation or admission.
Patients can also be referred from hospitals to support their discharge from inpatient services and from tertiary level outpatient services for patients that require further rehabilitation at a secondary level.
All referrals to see a community rehabilitation doctor (e.g. geriatrician, rehabilitation physician, falls specialist coordinators) must be sent through to the Department of Health's Central Referral Service (external site).

Community Rehabilitation Team
The Community Rehabilitation service is provided by a multidisciplinary team comprised of:
- clinic clerks
- clinical psychologist
- dietitian
- falls specialist coordinators
- geriatrician
- nurse
- occupational therapist
- physiotherapist
- rehabilitation physician
- social worker
- speech pathologist
- therapy assistant.
Note: your own GP will remain in charge of your care. You will need to contact your GP directly if you are in need of medical attention.
Group Work
The Community Rehabilitation team provides a range of group based activities.
Your therapist will refer you to these groups if these are appropriate to your rehabilitation goals.
Your journey with us
- Step 1: Referral to Community Rehabilitation.
- Step 2: Assessment to determine your needs and establish your goals.
- Step 3: Rehabilitation therapy - work together to meet identified goals.
- Step 4: Review with the team and alter care plan if necessary.
- Step 5: Discharge, with links to community services if required.
What to expect
You will be actively involved in identifying your rehabilitation goals/ priorities. Please bring some ideas to your appointments.
During your time with us you will be seen by more than one health professional. To ensure the care you receive is coordinated, you will be assigned a key worker within the team who you can contact if you have any questions.
You will be expected to make your own transport arrangements to appointments. If you have difficulty arranging this, please speak with your key worker or health professional.

- all aids (e.g. walking, hearing, reading glasses and splints)
- any home exercise programs
- current medication list.
