Diabetes - Adult Diabetes Service
The Armadale Health Service Diabetes Team provides:
- Diabetes Self-management education and
- Diabetes Multi-disciplinary Clinic
The Adult Diabetes Team is comprised of:
- Consultant Physician (see Diabetes Multi-Disciplinary-Clinic)
- Credentialed Diabetes Educators
- Senior Dietitians
- Podiatrist
Diabetes education
Intake criteria:
- Type 1 Diabetes (excluding patients on insulin pumps)
- Complex Type 2 Diabetes with at least 1 complication and/or HbA1c >9.0% (not newly diagnosed)
- Gestational Diabetes.
Individual appointments
Our diabetes educators and dietitians provide individual appointments for patients with Type 1 Diabetes, complex Type 2 Diabetes and Gestational Diabetes Mellitus. Please note there may be wait times for appointments.
Insulin therapy
Individual appointments are offered for people who are commencing on insulin therapy or need insulin change, adjustments or re-education.
Topics we cover during the education are:
- Insulin therapy, covering insulin administration and disposal of sharps
- Self-blood glucose monitoring
- Self-management of hypoglycaemia
- Problem-solving
- Dietary considerations.
If you require a diabetes educator to adjust insulin doses, please complete the Insulin Therapy Order Form (PDF 90KB).

Gestational Diabetes Group
This is a two-session education program for women diagnosed with Gestational Diabetes (GDM) to assist with the management of GDM and is run over 2 weeks.
Topics covered in this program include:
- What is Gestational Diabetes?
- Self-blood glucose monitoring
- Healthy eating
- Physical activity
- Recommended follow- up post gestational diabetes
Referrals and appointments
A referral is required from a general practitioner (GP) or a health professional to access the programs provided by the Armadale Health Service Adult Diabetes Team. When a referral has been received and accepted, the client will be contacted by phone or mail depending on urgency to be invited to attend the appointment.
Please specify in your referral if an interpreter is needed, is hard of hearing or has other disabilities.
How to refer
To refer a patient to the service, complete the diabetes education referral form (PDF 31KB) and email to AKG.Referrals@health.wa.gov.au or fax to 9391 1134.
Diabetes Multi-Disciplinary Clinic
Intake criteria:
- Complex Type 2 Diabetes with at least 1 complication and/or HbA1c >9.0% (not newly diagnosed)
The clinic is comprised of:
- Consultant Physician
- CredentialedDiabetes Educator
- Senior Dietitian
The multi-disciplinary clinic runs weekly on Friday from 12.30pm - 4.30pm.
