Collaborative care wins big at the Rotary Allied Health Excellence Awards 2017

Armadale Health Service’s Building Collaborative Care with Community Services (BCCCS) project has won the Excellence in the Community and Primary Care category in the 2017 Rotary Allied Health Excellence Awards.

Established in October 2016, the BCCCS working group is focussed on promoting shared care planning amongst health care givers, patients and their carers. The overall aim of this coordinated care approach is to achieve the best long term outcomes for patients.

The group includes allied health professionals, general practitioners (GP), nurses, community based health care service representatives, a consumer representative, and WA Primary Health Alliance (WAPHA).

Through a series of workshops, the group identified gaps and developed strategies to improve shared care planning to support these patients to manage their conditions in the community.

This resulted in the development of an online localised service directory (PDF 920KB) and a health professionals’ networking forum to encourage collaboration and communication between service providers.

A second forum​ has been planned for November 2017 aimed at introducing health professionals to community based services.

AHS Director of Allied Health Dr B-K Tan said the commitment of the group was key to the success of the project.

“Group member often demonstrated their dedication to the service by volunteering their time and efforts outside of their normal work commitments,” Dr Tan said.

“This included meeting regularly, sharing ideas and being involved in the implementation of strategies to improve collaborative care for patients.”

The working group is now developing a patient-focused education session and an online database of resources for patients with a chronic disease.

These initiatives are aimed at helping patients to better understand their diagnosis and manage their symptoms.

The team will now vie for the overall Allied Health Team of the Year honours at the awards ceremony on Monday 16 October. Good luck to the team!
