Rehabilitation and subacute unit

Our inpatient Rehabilitation and subacute unit provides a range of therapy and services for patients with cardiac, orthopaedic, respiratory, neurological and musculoskeletal conditions.

We provide rehabilitation services for anyone over 65 years of age within the Armadale Health Service Aged Care Client Record (ACCR) catchment areas (postcodes 6121 to 6126 and 6111 and 6112). Some patients under the age of 65 years may be cared for in this unit if they require rehabilitative care and will benefit from geriatric services.

We also provide inpatient services for Aboriginal and Torres Strait Islander (ATSI) patients over 45 years of age from the health service’s ACCR catchment area and some younger patients with geriatric syndromes.

The team works with the patient and their family/carer towards common goals to assist the patient to return home with appropriate resources or assist with the transition into residential care, should this be required.

Referrals

Referrals for patients requiring rehabilitation and subacute care are accepted from general practitioners (GPs).

All referrals must include reasons for admission, the patient’s medical history and current functional ability.
