Community Rehabilitation

Community Rehabilitation provides rehabilitative, aged care and specialist services to anyone aged 18 years and above living in Armadale and surrounding suburbs.

It aims to assist you to improve your functional capacity and maximise your independence.

This service is focussed on helping you to meet your rehabilitation goals within a timeframe. We will work with you to regain or retain certain physical and cognitive abilities that will enable you to remain a part of the local community.

Once you have completed your rehabilitative program with us, we will also link you in with other community organisations if you require further support.

Referrals to Community Rehabilitation

Community Rehabilitation accepts referrals for patients who require rehabilitative therapy to prevent hospital presentation or admission.

Patients can also be referred from hospitals to support their discharge from inpatient services and from tertiary level outpatient services for patients that require further rehabilitation at a secondary level.

All referrals to see a Community Rehabilitation doctor (e.g. geriatrician, rehabilitation physician, falls specialist coordinators) must be sent through to the Department of Health's Central Referral Service (external site).
